 HOSPITAL PALLIATIVE CARE
Our specialist palliative care consultation service at Hutt Hospital has been in place since 2007.
The team consists of a Nurse Practitioner, two Clinical Nurse Specialists and a Palliative Medicine Specialist.
The team is permanently based in the hospital, and sees hospital patients who are referred for
a specialist palliative care consultation. They provide palliative care advice to teams, nurses and allied health professionals in the acute setting, with a focus on those with complex care needs.
Over the past year, our hospital team implemented a rapid discharge process for patients who wished to die at home. This process focused on patient-centered care that ensured equitable, responsive, and proactive palliative care support throughout the transition.
WAIRARAPA PALLIATIVE CARE
Te Omanga Hospice continues to provide a specialist palliative care medical consultancy service to doctors and nurse practitioners in the Wairarapa, working alongside Kahukura, the Wairarapa Palliative Care Service. Each week one of our doctors travels to the Wairarapa to see patients in their own homes, including rural properties, Aged Residential Care facilities and in Masterton Hospital.
Our Medical team are on call 24/7 for all Wairarapa GPs, Nurse Practitioners and hospital doctors
for specialist advice on their patients’ palliative care needs.
