SCHEDULE 8.2 — FORM OF STATEMENT OF TRUTH
Rules 8.8 & 8.72

1. The form of statement of truth verifying a document specified in column 1 of the
following table is that specified in relation to it in column 2 of the table:

2. Where the statement of truth is contained in a separate document, the document

claim number. The document being verified should be identified in the statement of truth as
