Enter the number of regular meetings, and the number of emergency meetings held by the Conclave during the given period. Enter the meetings attended by the PSR.
In the Arrears drop down, select YES or NO, to indicate whether the PSR is in arrears to the Conclave.
Enter the PSR’s activities in the Remarks column.
Select the name of the SR and the Conclave Recorder, and submit the return.
