Enter the details of the remittance in the DETAILS OF REMITTANCE section of the Return Any Cessations or exclusions will be displayed in the FORM E section of the return.
Enter the reason for the Cessation / Exclusion.
Enter the date on which subscription was last received from the member.
You will need to upload the scanned copies of the notice served on the member, and the acknowledgement of receipt of the notice by the member, as a consolidated pdf file for each member.
