 How to Enroll
You will have an opportunity to enroll for benefits for a period of 45 days after your date of hire or at each annual enrollment period. You may not make changes to your benefits outside of the annual enrollment period unless you experience certain qualified changes in life status. Here’s a quick checklist to make the enrollment process as easy as possible:
Before You Enroll
ƒ Be sure to review your benefit information, especially concerning any new health plans.
ƒ Be informed so you can make the best benefit choices for you and your family.
ƒ Know what coverage you have now. Go to https://corizonhealth.benefitsnow.com, click on Quick Links and Your Benefits Summary in the yellow box to print a copy of your current benefit coverage statement. Having your current coverage and cost information handy will make it easy to compare your options and costs from year to year.
ƒ Review What’s Changing for the upcoming year, including your plan coverage levels and costs.
ƒ Create a list of current doctors and prescription drugs that you and your eligible dependents take. This will help you answer the Help Me Choose questions when you enroll.
When You Are Ready to Enroll
ƒ Log on to https://corizonhealth.benefitsnow.com to begin. If you have difficulty accessing your account, call the Corizon Health Benefits Service Center at 1-855-594-6218. Customer service representatives are available from 8 a.m. to 5 p.m. CT, Monday through Friday, to assist you.
– From the home page, click Enroll under Action Needed! Then click Continue to begin the process.
– From the Enroll in Your Benefits page, you’ll see all the benefits you choose. Click the Take Me Through Each Benefit button at the top right corner of the benefit grid. Or you can click on view/change to the right of each benefit line if you want to modify specific benefit elections.
– As you go through the enrollment process, you
can go back and make changes at any time by clicking Return to Benefit Summary. Even after you’ve completed your enrollment, you can come back to https://corizonhealth.benefitsnow.com
and make changes during the annual enrollment period. Any action you take on the enrollment site will automatically be saved. Whatever changes or elections you’ve made as of the enrollment deadline will be your coverage for the upcoming year.
– You can review and confirm your elections any time you’re on the site.
ƒ Don’t forget to use all of the decision support tools available as well as the Get Your Own Estimate (Ask Sara) feature at https://corizonhealth.benefitsnow.com to help you select the coverage that best meets your needs.
        You must actively enroll for all 2019 health coverage. Medical, dental and vision benefits will not carry over to the 2019 plan year. In addition, you must actively elect to contribute to the FSAs or the HSAs if you wish to make contributions to these accounts for the 2019 Plan Year.
