APPENDIX 10.1 – INSURANCE CLAIMS PROCEDURE
Once an accident has occurred this must be reported to O’Driscoll O’Neil within 30 days. To
request a claim form please complete the attached incident report form.
Once a claim form has been received all sections must be fully completed. Please note the
medical certificate cannot be completed by a physiotherapist.
When the claim form is fully completed and you have gathered all the relevant documents
please send all original copies to Vivienne Clarke, Hockey Ireland, Newstead Building, UCD,
Belfield, Dublin 4.
For Medical Expenses please ensure that you submit all the original receipts. If your receipt does not
provide sufficient details as to what treatment was carried out then an invoice supporting this will
also be required. Please note should the claimant have private health insurance they must submit a
claim to their private health insurers in the first instance. Any amount not covered shall be
considered by the insurers providing they are accompanied with a letter advising there is no
alternative cover in place. In relation to physio treatment this is only covered under the policy once
it has been referred by a medical practitioner.
For Loss of Income please forward payslips from the 3 months prior to the accident. You must
submit your claim to the social welfare to claim for an illness benefits that you are entitled to.
Confirmation as to whether you are or are not entitled to benefits must accompany your forms.
Insurers will also require a letter from your employer advising whether or not you are entitled to sick
pay.
