11. If you are operating as a “DBA,” here you will select the “SSN” button and type in your social security
number right below. If you are operating as a corporation (LLC, S-Corp, Corporation, Etc.) then you
would select “EIN” and insert your business EIN.
12. Unless your business is a unit of the Government (read description) you will select no for this question.
Although you may be pursuing Government contracts, you do not fit this criterion, unless you have
been official recognized by an official agency and cleared to operate as a extension of the government.
13. When selecting a designated contact person for your business, be sure to include:
a. A reliable person that will answer all forms of corresponded in a timely and professional
matter.
b. Correct phone number and email
