increments until you do see a change. You may also see a bit of erythema (reddening of the skin) around the treated area, and/or some oedema (swelling) – although this can take a few minutes to appear – see photo below. The important thing is to be patient...
What are the effects of too much fluence (energy)?
1. More damage to the collagen resulting in more scarring
2. More scarring means more scattering in subsequent treatments
3. More scattering means less effective treatments, especially in
deeper regions
4. Less likelihood of a successful clearance!
Some people look for a definite ‘white’ frosting, or are listening for a loud ‘crack’ sound. This is wrong! In fact, in many cases, a clear white frosting or loud crack indicates that too much energy has been delivered into the skin!! In such cases, the initial energy will be absorbed by the ink and generate the desired reaction. However, any excess energy (which generates the white frosting dots or sonic cracks) will also induce possible scar tissue formation, in time.
(I have seen videos where the laser operator has pulled the laser tip away from the skin until they hear a ‘crack’ sound! This means that they are applying a very small spot size at the skin which means a large fluence – which means a high risk of damage. Please don’t do this!!)
Mike wrote a blog post with three videos discussing this ‘chasing the crack’ phenomenon.
Figure 21:Frosting appears soon after the laser is applied. Shortly after, the tattoo appears ‘bruised’ with some localised redeening and swelling.
