    Figure 23: This ‘crab’ tattoo had a little red in it, which turned black after the first laser treatment (with a ruby laser). We continued treating it and after 12 sessions achieved the final result on the right. This was a particularly tricky tattoo due to the intense density of the black ink.
The laser treated tattoo ink can be thought of as being removed in a ‘layer by layer’ fashion. The first treatment removes the topmost layer, with the second treatment attacking the newly exposed top layer.
When your patient/client returns for subsequent treatments you will need to increase the applied fluence since you will be targeting deeper ink than previously. As above, increase the fluence by increments of 0.5 J/cm2. Remember, scattering causes the laser beam to ‘spread out’ as it goes deeper into the skin, thereby falling in fluence.
By using this protocol, you should ensure minimal tissue damage in the dermis, thereby reducing the risk of scarring.
