Matching Donations from $100 to $1,000

to the Annual Fund and/or Polio Plus.

How it Works

1. Write a personal check made payable to The Rotary Foundation

in any amount between $100 and $1,000.

2. Complete the form below indicating your club and fund you want
to contribute to.

3. Mail check and completed form to:

All contributions must be postmarked by February 28, 2019 to be

eligible for matching points. Please allow a couple weeks for points
to be transferred. Once points are transferred, Rotarians can submit

recognition request through your Club Foundation Chair. Only 50,000
points available, so act quickly and don’t miss this opportunity.
