APPOINTMENT OF ENDURING GUARDIAN
                                (pursuant to section 6 of the Guardianship Act 1987 NSW)


            1.     Appointment of enduring guardian

                   I,      Name:           ESTHER ANNE JONES
                                           (otherwise known as ESTHER ANNE SCOTT and referred to as
                                           ESTHER ANNE JONES in the  remainder of the document)
                                           (“the Appointor”)

                           appoint Name:  GARY DAVID JONES

                           Address:        279 Coal Point Road, Coal Point NSW 2283

                           to be my enduring guardian.


            2.     Alternative guardian

                   I also appoint:

                           Name:           AMIE LOUISE WARD

                           Address:        3 Amber Grove, Bolwarra Heights NSW 2320

                           to be my alternative enduring guardian.


            3.     The appointment of an enduring guardian will be vacated in the following circumstances:

                   a.      if  the  enduring  guardian  dies  (proof  of  which  shall  be  evidenced  by  a  Death
                           Certificate);
                   b.      if the enduring guardian receives notice in writing that their appointment has been
                           revoked (proof of which shall be evidenced by a document signed by the Appointor
                           to that effect);
                   c.      if the enduring guardian resigns (proof of which shall be evidenced by a document
                           signed by the enduring guardian to that effect);
                   d.      if the enduring guardian, by reason of any physical or mental incapacity, ceases to
                           have the capacity to continue to act as an enduring guardian (proof of which shall
                           be  evidenced  by  a  medical  certificate  issued  by  the  enduring  guardian’s  treating
                           medical practitioner to that effect) or is a person whose personal estate is liable to
                           be dealt with in any way in accordance with the law relating to mental health (proof
                           of which shall be evidenced by a Court order or other document to that effect); or
                   e.      in such other circumstances as prescribed by the Guardianship Act 1987.
