An inspection of the property identified above on 4/14/2015 has established that violation(s) of City of Tampa Code section(s) 19-49, 19-56, and 19-233 exist on the property. Pursuant to Tampa City Code section 19-5(a)(7), the Director has ORDERED that the following action be taken by July 1, 2015:
Remove all junk, trash and debris from the entire property, including the rear of the property; Remove all inoperative vehicles. Remove all junk, trash and debris from the right of way. Secure the property in accordance with Section 19-233.
If the violations are not abated by July 1, 2015, the Director will authorize corrective action to enter onto the property to abate the violations and bill for the actual costs or funds expended by the City, which may include an administrative fee associated with the abatement of the violations. The bill shall be immediately due and payable upon receipt.
Failure to pay the bill for abatement upon receipt will result in a special assessment lien being recorded against the property. This special assessment lien will be superior to all other liens except prior liens for taxes. Upon recordation of the special lien will begin incurring simple interest at the rate of one-half (1⁄2) percent per month on the unpaid balance.
This order may be APPEALED to the City of Tampa Special Magistrate pursuant to Section 9-19 by July 1, 2015 by filing written notice with the Secretary at 315 East Kennedy Blvd., 3rd Floor, Old City Hall, Tampa, Florida 33602, (813) 274-7064 and serving a copy thereof on the Director at 2105 North Nebraska Ave., Tampa, Florida 33602, (813) 274-5545. Conveyance of your interest in the property while this Order is pending will not affect this Order. Any person acquiring an interest in the property takes it subject to this Order.
Verizon Florida LLC Lifeline Service in Florida
Lifeline is a government assistance program that is supported by the Florida Public Service Commission and the Federal Communications Commission. The Lifeline program provides assistance to reduce the basic monthly telephone rate by $15.91 for eligible residential consumers. As an Eligible Telecommunications Carrier, Verizon Florida LLC (Verizon) offers the Lifeline discount on Lifeline supported services, which include basic local telecommunications service and a number of residential bundled packages.
Only eligible consumers may enroll in the program. You may qualify for Lifeline service if you can show proof that you par- ticipate in certain government assistance programs or your annual income is 150% or below the Federal Poverty Guide- line. If you qualify based on income, you will be required to is limited to one discount per household consisting of either wireline or wireless. Therefore, you are required to certify and agree that no other member of the household is receiv- ing Lifeline service from Verizon or another communications provider. Verizon also provides Lifeline Service to residents of federally recognized lands who meet Native American Consumers who willfully make false statements in order to or may be barred from the program.
You must meet certain eligibility requirements in Florida in order to qualify for Lifeline Service. An Application for Verizon Lifeline Service can be obtained by contacting Verizon at www.verizon.com/lifeline or by phone at 1 800 VERIZON.
