ESSILOR EXPERTS™ PARTICIPATION AGREEMENT

On average, an ECP with a contract that is fully completed and executed on the first try will receive a Welcome Call 7 days
sooner. To help ensure your ECPs are moving through the Essilor Experts process optimally, please be sure to complete
all activities on the checklist contained within the latest version of the EE contract.

EYE CARE PROFESSIONAL CHECKLIST

Thank you for your interest in Essilor Experts™. In order to ensure timely and accurate processing of
your Essilor Experts™ Participation Agreement we encourage you to use the following checklist as you
review and complete the Agreement.

 Review the complete practice information section in the header of the Agreement. All lines
should be completed and accurate.

 Complete the Salutation Line with your name

 Complete the line next to “Eligible Practice” with your practice name.

 Review each page of the Agreement and initial at the bottom right.

 Complete Schedule a table if you have additional practice locations that will be covered under
this agreement.

 Sign and complete the information in the “Agreed To and Accepted” Section on page 1.

 Return the executed Agreement to your Essilor representative.
