Category 1: Organization, Responsibility and Control
1.1 The Provider shall be incorporated, registered or otherwise recognized
as a legal entity.
I5O Consulting Services is a Limited Liability Company formed in the year 2018. The company is
currently in compliance with all required state and federal filings. The organization is led by Dr.
Teresa Hardee, EdD, CPA, SSBB who brings over 20 years of accounting, auditing, budgeting,
executive leadership in higher education and management experience. Refer to the I5O
Consulting Services Articles of Organization and Certificate of Formation documents for
supporting documentation.
