The most important is the Hippa compliant parental consent form and a template is available that can be customized with your clubs’ information. You must have a signed consent form before you screen a child. These forms can be given to the daycare center for distribution prior to the screening day. There is also a form that the school should fill out with the child’s information that can be pre-loaded into the Spot device before the day of the event.
Screening results in most cases can be downloaded to the school administration so that they can disburse this information to the parent; however, each device has a wi-fi capable printer available if you choose to print the results out for the parent. This would most likely occur at a community event such as a health fair.
Apart from the Dallas Filipino Lions Club, we are all fairly new to this and learning as we go, but there is enough information, training and support that I feel certain that each club in District 2-X1 can have a successful Lions Kidsight USA screening event. Twenty-seven years ago, the Coppell Lions Club helped develop and build a "Welcome to Coppell" sign on Belt Line Road just north of Highway 635. Over the years, the sign began to show its age. It needed to be refreshed. The Coppell Lions Club worked with
the City of Coppell to implement a plan to clean and paint the sign as one of their Legacy Projects. They also replaced the Lions sign that accompanied the city sign. The work was completed in April of this year. The accompanying pictures show the before and after results of the project. It looks so good they probably should go into business across the community cleaning up the city one sign at a time. Hope the other clubs in the district get the idea of how to make an impression in your community.
