Pre-Assessment

 Introduce yourself to patient

 Perform hand hygiene

 Check home/room for contact or airborne precautions

 Explain OASIS comprehensive assessment to patient

 Be organized and systematic in your assessment

 Use appropriate listening and questioning skills

Contact Precaution: If performing a SOC, information can be obtained
from patient or family members or discharge instructions. If the visit is a
revisit, clinician will observe special equipment in the home for vital signs
and also in the patient's chart
