Funeral Wishes Form

In my role as a celebrant, visiting bereaved families, it has become apparent to me that for
some families and friends, arranging a loved one's funeral service is one of the hardest things
they have ever done. especially if there is little knowledge about the loved one's personal
details and preferences. It is a huge responsibility and, understandably, those arranging the
funeral want to get it right which can cause them stress, arguments between family members,
etc.

So I have put together a simple form where you can record your wishes, your preferences, your
likes and dislikes, a little background about yourself, to make it easier on your loved ones. Just
print out the next few pages, complete them and leave them somewhere safe or give them to
your next of kin to use when your time comes. You can also request a separate copy by email.
