GRADE DESCRIPTION
0 Intact mucosa with a completely normal appearance
and no areas of hyperkeratosis
1 The mucosa is intact but there are areas of
hyperkeratosis
2 Small (<2 cm) single ulcer or smaller multifocal
ulcers
3 Large (>2 cm) single ulcer or large multifocal
ulcers
4 Extensive ulceration that may involve coalescence
of multiple ulcers. Bleeding may be evident
