Triple-ligate the pulmonary arterial supply and transfix with 3 metric (2/0 USP) or 2 metric (3/0 USP) non-
absorbable suture material.
2 Transect the pulmonary artery between the two distal sutures.
3 Carefully dissect the venous supply and ligate in a similar fashion.
4 Dissect the bronchus free and transect it between two pairs of crushing forceps.
PRACTICAL TIP
Sterile cotton-tipped applicators can be useful for gently teasing and
bluntly isolating the artery, vein and bronchus
5 Remove the affected lobe.
6 Preplace a series of interrupted horizontal mattress sutures (3 metric (2/0 USP) or 2 metric (3/0 USP) non-
absorbable suture material) in the bronchus before placing sutures to seal the bronchus.
7 Oversew the distal tissue using a simple continuous pattern (1.5 metric (4/0 USP) or 2 metric (3/0 USP) absorbable
suture material).
Preplacement of interrupted horizontal mattress sutures in the bronchus. The mattress sutures are then tied and
the end of the bronchus is oversewn using a simple continuous pattern.
