A FREE opportunity to learn about Health Care Power of
Attorney and Living Will forms.

Think about it. Talk about it. Write it down.
At our virtual workshops, trained facilitators will provide information
about advance care planning and will walk you through the Health
Care Power of Attorney and Living Will documents. This information
will guide you through the steps for getting the medical care you want
if you are unable to speak or make decisions about your treatment.
