High power view of damaging infiltrate with two or more foci of myocyte damage.
Grade 3R

Diffuse process of myocyte injury

Treatment:
Ensure below has been obtained or started within the last 24-48 hours
 HLA antibody profile (PRA) and lymphocyte subsets
 Renal profile, glucose, CMV PCR, EBV PCR
 Initiate Proton Pump Inhibitor GI prophylaxis
