Congratulations!  MOCO Movement Center offers Voluntary Employee Benefits through
               Allstate Benefits! This packet contains brochures for all the voluntary benefit choices
               available to you. Most of these voluntary benefits are employee funded through payroll
               deduction and are completely voluntary. Your employer pays for the Accident Plan for
               you! By enrolling through your company, you get greatly reduced “group rates”, pre-tax
               payroll deduction status for many of the benefits (which saves you an additional 25-
               30%) and the convenience of having the premiums come right out of your paycheck.
               Once you get them, they are yours, and you can keep them - at the same rates - for as
               long as you like, even if you retire or change employers.

               Available benefits include:

                   •  Short-Term Disability Insurance – Paycheck Protection if you get sick or
                       injured and cannot work (includes Maternity Leave Benefits for Women)
                   •  Term & Whole Life Insurance for yourself, your spouse, and your children
                   •  Supplemental Insurance Plans (to help with lost income, deductibles, co-pays,
                       and out-of-pocket medical expenses):
                          o  Hospital indemnity Plan
                          o  Accident Plan
                          o  Cancer Plan
                          o  Critical Illness Plan

               Please note that many benefits are initially offered on what is called a “Contingent
               Guaranteed Issue” basis (only 2 medical questions) which makes qualification relatively
               easy for almost everyone. But these relaxed qualification criteria are only available
               to you during your initial eligibility period.  If you decline coverage at that time, and
               later decide you want to enroll, you will have to qualify by answering more extensive
               medical background questions, the answers to which will determine if you are accepted
               or declined.  So, please make sure you look at these benefits and take advantage of the
               relaxed underwriting period if you are interested – especially if you currently have any
               medical conditions.
