               5.0    DEFINITIONS

                                                      Accident Classification

                     1)    Near Miss

                           "An event (accident) where no injury or damage occurred

                           e.g. someone tripped over the hazard but suffered no injury. Any incident which could have
                           resulted in any of 2 to 10 below.

                     2)    Damage Only

                           "An event (accident) where only property damage has occurred"

                           e.g. someone loses control of large cable drum which careers into another object and breaks
                           it.

                     3)    Accident Book Entry - Minor Injury

                           "Any personal injury accident that may or may not require First Aid treatment"

                           Not falling under any category below, which does not result in time off work on the day/shift
                           when the accident occurred.

                     4)    Medical Treatment

                           "Any injury or illness requesting qualified medical treatment"

                           Treatment means surgery, stitching of wounds, administration of prescription only medicine
                           by a doctor, or a nurse under guidance of a doctor.  Normally provided off site.  It does not
                           include precautionary x-ray or examinations.

                     5)    Restricted Work Condition

                           "Where an employee returns, or remains at work but is unable to undertake the full
                           range of duties or normal hours of work"

                           Following an accident or illness an employee is unable, or on medical advice is prevented
                           from, doing the full range of work or working normal/shift hours but can undertake mutually
                           beneficial duties.

                     6)    Lost Time Accident

                           " 1, 2 or 3 days absence following the day of the accident"

                           Accidents will be recorded as LTA's which result in the injured person being incapacitated for
                           normal work duties for the following periods of time. (Except where it falls within
                           classifications 7 or 8 below)
                           a)  One day/shift following the day of the accident
                           b)  Two days/shifts following the day of the accident
                           c)  Three days/shifts following the day of the accident

                           (For statistical purposes this category does not include "over three day accidents" to avoid
                           duplication)
