VOLUNTARY LONG-TERM CARE
Voluntary Long-Term Care insurance is available through Unum. Long Term Care insurance provides a benefit when someone
needs help with two or more activities of daily living, such as dressing, bathing, eating or moving about – or when someone
suffers severe cognitive impairment. No evidence of insurability is required up to certain guaranteed issue amounts, and
the policy is portable, so you can take it with you if you leave the company. If you are a late entrant, you must complete the
evidence of insurability form to enroll in the plan. See the Unum Long-Term Care packets for age based program pricing.

TRAVEL ASSISTANCE
QSC offers Travel Assistance to employees and their family members through Cigna. This plan provides you with support
in the case of a medical emergency while you are traveling on business for QSC or for personal reasons. A full range of
services are available to assist you during simple to extreme travel emergencies, such as medical consultation / evaluation
/ referral for emergency situations, hospital admission assistance, emergency medical evacuation, critical care monitoring,
medical repatriation, prescription drug assistance, emergency messaging, and more. This coverage is provided by QSC at
no cost to you.

VOLUNTARY LEGAL PLAN
As a Legal Shield member, you, your spouse and your children are covered for a wide variety of legal issues and concerns.
You may also purchase Identity Theft Shield, which includes regular monitoring of your credit and assistance with identity
theft. Specific program pricing details are located in the “Your Cost for Coverage” section of this guide.
