EMPLOYEE ASSISTANCE PROGRAM (EAP)




        The Employee Assistance Program (EAP) through Cigna provides employees and their household members with free,
        confidential assistance to help with personal or professional problems that may interfere with work or family responsibilities.
        Services are available 24-hours-a-day, 7-days-a-week via a toll-free nationwide number. Employees and their household
        members can receive up to 6 face-to-face sessions per member per issue per year at no charge. The EAP offers short-term
        counseling for a wide range of emotional health, family and work-related issues with telephonic and online worklife resources.
        Services include resources for relationship difficulties, emotional/psychological concerns, work or family stress/anxiety,
        alcohol/drug abuse, personal/life improvement, legal or financial topics, depression/grief, childcare, eldercare, adoption, pet
        care, daily living issues and other issues that may occur.


        In addition, your EAP benefit offers legal and financial guidance from qualified professionals, including an initial 30-minute
        consultation for certain issues. Typical financial guidance resources include credit counseling, debt / budgeting assistance,
        tax planning, and retirement and college planning.

                       Log in to www.cignabehavioral.com and enter the employer ID (qsc) or call (877) 622-4327 to be
                       immediately connected to an EAP counselor.




        BASIC LIFE AND ACCIDENTAL DEATH &

        DISMEMBERMENT (AD&D)




        Providing economic security for your family if you die, become disabled, or experience an injury or illness is a major consideration
        in personal financial planning. QSC provides you with Basic Employee Life and Accidental Death and Dismemberment (AD&D)
        insurance coverage through Cigna at no cost to you. You automatically receive Life and AD&D coverage in the amount of
        one times your annual salary up to $400,000. You must choose a beneficiary to receive benefits in the event of your death.



                   Consider updating your beneficiary designation if you have experienced a life changing event such as marriage,
                   divorce, the birth of children, etc.
