Voluntary Worksite Benefits
Kate Somerville makes a broad array of voluntary benefits available to you through UNUM. These voluntary coverages have unique
features that are highlighted below:
• Most benefits paid directly to you unless otherwise specified.
• Individual coverage purchased through payroll deductions. The coverage stays with you beyond retirement.
• Most plans pay benefits regardless of other insurance coverage.
Choose from the following plans according to your needs:
VOLUNTARY ACCIDENT
Kate Somerville provides employees the opportunity to apply for Group Accident Insurance through Unum on an annual basis. Group
Accident pays benefits to help you cover the unexpected costs associated with accidental death or injuries. Benefits are flat dollar
payments and are not based on medical expenses incurred. You may also apply for coverage for your spouse/registered domestic
partner and your dependent children. This benefit is 100% employee paid. Your benefit counselor will be able to confirm your per
paycheck cost as rates vary.
VOLUNTARY CRITICAL ILLNESS WITH CANCER
You have the opportunity to apply for Critical Illness Insurance on an annual basis, a limited benefit policy offered through Unum.
This insurance is designed to help you offset the costs of certain covered critical illnesses such as stroke, permanent paralysis, heart
attack, end-stage renal disease, and cancer. When diagnosed with a covered condition, you will receive a lump-sum benefit that
you can use for expenses such as deductibles, copays, transportation, and so on. Your benefit counselor will be able to confirm your
per paycheck cost as rates vary.
VOLUNTARY SHORT-TERM DISABILITY (STD)
Short-Term Disability insurance assists in replacing a portion of your income if you become unable to work because of a covered
accident or illness. Employees may choose the amount of disability benefits to meet their personal needs. With UNUM, you may
customize your Short-Term Disability plan according to what best fits your needs. Please note that if you wish to enroll in this plan,
you must do so via UNUM’s website. Your benefit counselor will be able to assist with plan options and rates.
VOLUNTARY HOSPITAL INDEMNITY
This benefit is designed to help provide financial protection for covered individuals by paying a benefit due to a hospitalization.
Employees can use the benefit to meet the out-of-pocket expenses and extra bills that can occur. Indemnity lump sum benefits are
paid directly to the employee based on the amount of coverage listed, regardless of the actual cost of treatment. Your benefit
counselor will be able to assist with plan options and rates.
YOUR UNUM PLAN PAYS A WELLNESS
BENEFIT FOR ONE WELLNESS TEST EACH
YEAR.
With Unum’s Wellness Benefit, you and other covered
family members can receive a valuable incentive for
important tests and screenings. Many of these tests
are routinely performed, so it’s easy to take
advantage of this benefit. Your Critical Illness
wellness benefit is $75. Your Accident wellness
benefit is $50.
