Bass is currently enrolled in the HMO FULL plan.

He wants to know if there is a plan that offers
flexibility and saves him money.
(HMO FULL plan benefits are changing this year)

Bass decided to make the move from the HMO FULL plan to the HSA PPO plan. Besides
Bass saving money, he will have access to additional benefits offered by QSC to work
with the HSA PPO plan, listed below.

• QSC will automatically setup the HSA Banking account through Discovery Benefits
that Bass will use with the HSA PPO plan

• Bass will receive a debit card and he can download the Discovery Benefits Mobile
App and manage the money in his account to pay for medical, dental or vision
services

• Bass can request access to his future HSA contributions through Discovery
Benefits’s HSA Advance feature to have peace of mind that his immediate health
care expenses are covered while his HSA balance grows

• Bass can download the My Cigna App to find out how much medical or dental
services will cost and shop for the best price. Bass can also shop for his
prescriptions on Good Rx or One Rx for the best cost for his prescription

This was a lot of information to consider, but he is excited that he is in charge of his
current health care expenses and saving for his future!
