Aetna offers a secure, private, online resource that makes it easy for you to view, access and manage your health information, as
well as share it with your doctors.
• Keep your health information in one place: It’s always available for you to access, including in an emergency.
• Share your history with your doctor: Print your Health Summary and take it with you to doctor visits. Give your doctors a
more detailed view of your health, either printed out or online.
• Stay aware of your health needs: Based on your health profile, the Personal Health Record may send you health-related alerts
and reminders to help you address your health needs in a timely manner.
• Track important health numbers: Use built-in Health Trackers that show you how your health data changes over time in clear,
easy-to-understand graphs and charts. Keep track of your blood pressure, blood sugar, weight and more.

Aetna | Value Added Programs

As an Aetna member, you are offered the following programs and services at no cost to you:
• 24/7 Nurse Line: You can call the 24/7 Nurseline anytime and talk to a registered nurse for answers to
health-related questions. You can also listen to information from Aetna’s audio health library on
thousands of topics.
• Teladoc: Teladoc is a national network of board-certified physicians who provide quality health care
through the convenience of phone or online video consultations. Teladoc physicians can diagnose, treat
and write prescriptions for routine medical conditions.
• Beginning Right Maternity Program: This program provides services, information and resources to help
improve pregnancy outcomes. Nurse outreach to physicians is offered for high-risk members.
• Chronic Kidney Disease Program: This program was launched to improve clinical outcomes and reduce
medical costs by slowing the progression of chronic kidney disease. This program may include
telemonitoring equipment, support from a kidney healthcare management team, coordination with
your doctor, 24 hour access to kidney nurse and personalized information.
• Aetna Health Connections - Disease Management with MedQuery: This program supports more than 35 chronic conditions.
Integrated and personalized patented technology allows Aetna to tailor your interactions based on health and disease states,
benefit plan coverage and personal preferences. MedQuery with Member Messaging is a program that uses medical claims,
pharmacy claims and lab result values at a member level, compares that against complex algorithms developed from evidence-
based standards of care to identify potential gaps in care called Care Considerations and provides the care considerations to
you and your physicians to help improve your patient care.
• Health History Report: This report provides a centralized summary of your health-related activity, such as doctor visits, tests,
treatments, and prescripti8ons for medications.
• Enhanced Clinical Review: This program can limit the financial impact of high cost radiology services, diagnostic cardiology,
sleep management studies, hip and knee arthroplasties and cardiac rhythm implant devices by coordinating information
provided by your doctor.
• Simple Steps to a Healthier Life: This is a personalized online health and wellness program that begins when you complete a
health assessment. You’ll receive a Health Summary Report to keep and record your results over time, which can also be
printed and shared with your doctor. Based on information gathered in the health assessment, you receive a personalized
HealthMap, containing online coaching program recommendations to help you achieve and maintain good health. The Health
Assessment is also designed to assess your level of health risks, your readiness to change certain health behaviors and your
impact of health on productivity.
• Aetna Vision Discount Program: This program helps you save on many eye care services and products, including eye exams,
LASIK surgery, eyeglasses, contact lenses, nonprescription sunglasses, contact lens solutions and other eye care accessories.
