DISABILITY INSURANCE
ANTHEM BLUE CROSS | VOLUNTARY SHORT TERM DISABILITY
California Employees Only
If you experience a Short Term Disability claim, you are eligible for benefits provided by the state. California State Disability Insurance (SDI)
is a partial wage-replacement insurance plan for California workers. The SDI program is state-mandated and funded through employee
payroll deductions. SDI provides short-term benefits to eligible workers who suffer a loss of wages when they are unable to work due to a
non-work-related illness or injury, pregnancy or childbirth. For more information, please visit the EDD website at www.edd.ca.gov.
Milani offers you the opportunity to purchase additional Short Term Disability (STD) to provide income replacement if you become disabled
due to accident, sickness or pregnancy, through Anthem Blue Cross. If you experience a temporary disability, benefits begin on the 1st day
for an accident and on the 8th day for an illness of your disability and will continue as long as you remain disabled up to a maximum of 13
weeks. This STD plan provides you with a monthly benefit equal to 20% of your pre‐disability earnings up to a maximum benefit of $1,000
per week.

Employees Residing in all Locations other than California
Milani offers you Short Term Disability (STD) to provide income replacement if you become disabled due to accident, sickness or
pregnancy, through Anthem Blue Cross. If you experience a temporary disability, benefits begin on the 1st day for an accident and on the
8th day for an illness of your disability and will continue as long as you remain disabled up to a maximum of 13 weeks. STD works with
state disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal to
67% of your pre‐disability earnings up to a maximum benefit of $2,500 per week.
ANTHEM BLUE CROSS | LONG TERM DISABILITY
Milani offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an extended period of time,
through Anthem Blue Cross. If you become totally and permanently disabled, benefits begin 90 days after the start of your illness or
injury and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD works with state disability
programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal to 60% of your
pre‐disability earnings up to a maximum benefit of $10,000 per month.

The Employee Assistance Program (EAP) provides you and your household members with free, confidential assistance to help with personal
or professional problems that may interfere with work or family responsibilities and obligations.
You and your household members can receive:
24/7 phone consultations with licensed mental health professionals and referrals to supportive resources.
Up to 3 counseling sessions, face-to-face or online using LiveHealth Online, for each concern.
Online programs to offer something different than traditional counseling.
Access to quick and confidential help from legal and financial experts.
