VOLUNTARY PET HEALTH CARE PROGRAMS

Discount Plan

With the United Pet Care (UPC) discount plan, you can reduce the cost of your veterinary bills by saving up to 50% on each
veterinary visit for your pet when you visit a contracted United Pet Care provider. This plan offers discounts for office visits,
procedures, medications, and more. With UPC’s program, there are no deductibles, no claim forms, no exclusions due to
pre-existing conditions or age, no annual dollar limitations, no waiting periods, and no limit on the number of visits annually.
You may pay for this benefit through salary deduction (paycheck)

Reimbursement Plan
With the Nationwide reimbursement plan, you can visit any licensed veterinarian, veterinary specialist or animal hospital in
the United States. To receive reimbursement through Nationwide, fax or mail their easy-to-use claim form along with your
invoices. After meeting your policy’s deductible, you will be reimbursed according to your plan’s benefit schedule allowance
or the invoice amount, whichever is less. If elected, you will own this policy and pay premiums to Nationwide directly.

SUPPLEMENTAL BENEFITS

You may purchase individual policies from Cigna to help cover what other insurance doesn’t. If elected, your premiums are
paid through payroll deductions on an after-tax basis.

Critical Illness
Cigna’s Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-
threatening illness. You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as invasive cancer,
a heart attack, stroke, kidney failure, paralysis, blindness, and more.

Accident
Accidental injury coverage, offered by Cigna, provides a cash benefit if you suffer certain injuries, undergo specific treatments,
or incur other itemized expenses resulting from an accident. Cash payouts are available for initial and emergency care,
hospitalizations, fractures and dislocations, and follow-up care.

Hospital Care
Cigna’s Hospital Care coverage provides a fixed cash benefit according to a schedule when you incur a hospital stay resulting
from a covered injury or illness.
