VOLUNTARY SHORT-TERM DISABILITY
QSC offers you the opportunity to purchase Short Term Disability (STD) at discounted group rates to provide income
replacement if you become disabled due to accident, sickness or pregnancy. If you experience a temporary disability, benefits
begin 7 days after the start of your disability and will continue as long as you remain disabled up to a maximum of 13 weeks.
STD works with state disability programs, Social Security, and any other group disability coverage, to provide you with a
combined monthly benefit equal to 60% of your pre-disability earnings up to a maximum benefit of $2,500 per week.
Please note that benefits for new hires wiII not be payable for a disability within the first 12 months of the plan if you have
been treated, consulted or diagnosed for a pre-existing condition within 3 months prior to your effective date of coverage.
LONG-TERM DISABILITY
QSC believes that Long-Term Disability (LTD) coverage is important because anyone at any age may become injured or ill for
an extended period of time. Our LTD coverage through Cigna will replace 60% of your pre-disability earnings to a monthly
maximum of $10,000 if you are disabled for more than 90 days and are unable to work. LTD benefits are offset with other
sources of income, such as Social Security and workers’ compensation. You are automatically covered under the LTD plan
at no cost to you.

EMPLOYEE ASSISTANCE PROGRAM (EAP)
The Employee Assistance Program (EAP) through Cigna provides employees and their household members with free,
confidential assistance to help with personal or professional problems that may interfere with work or family responsibilities.
Services are available 24-hours-a-day, 7-days-a-week via a toll-free nationwide number. Employees and their household
members can receive up to 6 face-to-face sessions per member per issue per year at no charge. The EAP offers short-term
counseling for a wide range of emotional health, family and work-related issues with telephonic and online worklife resources.
Services include resources for relationship difficulties, emotional/psychological concerns, work or family stress/anxiety,
alcohol/drug abuse, personal/life improvement, legal or financial topics, depression/grief, childcare, eldercare, adoption, pet
care, daily living issues and other issues that may occur.
In addition, your EAP benefit offers legal and financial guidance from qualified professionals, including an initial 30-minute
consultation for certain issues. Typical financial guidance resources include credit counseling, debt / budgeting assistance,
tax planning, and retirement and college planning.
