Finding Participating Dentist in the MetLife DHMO/Managed Care Plan
The Dental HMO/Managed Care plan’s network includes both private practice dentists and those who are in a clinic environ-
ment. You can find the names, addresses, languages spoken and phone numbers of participating dentists by searching our
online Find a Dentist directory.
Step 1: Go to www.metlife.com/dental
Step 2: Select Find a Dentist
Enter your ZIP Code and select the Dental HMO/Managed Care network.
Step 3: Enter the Plan Name MET245
The plan name is located in your Schedule of Benefits.
Finding Participating Dentist in the MetLife PPO Plan
Step 1: Go to www.metlife.com/dental
Step 2: Select Find a Dentist
Enter your ZIP Code and select the PDP Plus network.
Step 3: Advanced Search
Use the Advanced Search option to locate a dentist by name, language spoken,
specialty or gender.
