TASC | COMMUTER BENEFIT PLAN
United Capital provides you with a Transportation Spending Account Plan through TASC. The Transportation Spending Account Plan
allows you to pay for work-related transit and parking expenses with pre-tax dollars. You can set aside up to:
z $255 per month for mass transit expenses including Transit Passes and Vanpooling
z $255 per month for parking
TASC offers an online order model which allows you to place your respective parking and transportation orders via the TASC website.
Through the TASC website, you will have the opportunity to place your monthly order during the defined order period.
SUPPLEMENTAL BENEFITS
You may purchase Supplemental Benefits from Assurant/Sun Life including Voluntary Accident, Voluntary Critical Illness, and GAP
Insurance. Your premiums are paid through payroll deductions on a after-tax basis. Assurant/Sun Life policies offer direct-to-the-
policyholder cash payouts to help cover what other insurance doesn’t. Please contact your HR Representative for benefit details
and rates.
ASSURANT/SUN LIFE | VOLUNTARY ACCIDENT
Accident insurance will pay you or a covered family member a benefit to help cover your out-of-pocket medical costs in case of an
accident. For covered accidental injuries, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid
according to a fixed schedule that includes benefits for hospitalization, fractures and dislocations, emergency room visits, major
diagnostic exams, physical therapy, and more. In addition, you are eligible for a $50 wellness reimbursement for obtaining any one
wellness screening from a list of more than 20 covered tests.
ASSURANT/SUN LIFE | VOLUNTARY CRITICAL ILLNESS
Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-threatening illness.
You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as a heart attack, stroke, kidney failure and
cancer. In addition, you are eligible for a $50 wellness reimbursement for obtaining any one wellness screening from a list of more
than 20 covered tests.
If you are in your initial eligibility period, guarantee issue is available to you in amounts up to $15,000. If you are no longer in your
initial eligibility period, you may enroll in Voluntary Critical Illness as long as you provide proof of good health. To provide proof of
good health, you will be asked to complete a health questionnaire and are subject to insurance carrier approval. Assurant/Sun Life
may approve or decline coverage based on a review of your health history.
ASSURANT/SUN LIFE | GAP INSURANCE
GAP insurance is designed to supplement your health insurance plan by providing lump-sum benefits to cover eligible out-of-pocket
medical costs for you and your covered family members. Benefits may help cover expenses related to coinsurance, copays, and
deductibles for inpatient and outpatient services. This plan provides benefits up to $2,000 per member per calendar year for inpatient
hospital stays, and $1,000 per member per calendar year for outpatient procedures.
