Teladoc is a convenient and affordable alternative to Emergency Room and Urgent Care visits that gives you the ability to
resolve many common medical issues 24 hours a day, 7 days a week, 365 days of the year by phone, mobile app or online
video. You or your covered dependents enrolled in any Blue Shield medical plan have access to this service.

Contact Teladoc from home, work, or on the road and let the doctor come to you. Teladoc doctors diagnose non-emergency
medical problems, recommend treatment, and can even call in a prescription to your pharmacy of choice, when necessary.

Teladoc doctors are U.S. board-certified, licensed in your state and average 20 years of practice experience. Teladoc does not
replace your primary care physician. With your consent, Teladoc will provide information about your consult to your primary
care physician.

Benefit Highlights
• There are no patient age limitations. Teladoc can treat all patients including newborns.
• Teladoc consults are available wherever the patient is - at home, at work, or travelling within the US.
• The telephonic and/or video consultation includes the diagnosis and recommended treatment. A short-term non-DEA
controlled prescription will be ordered when appropriate.
Cost: Trio HMO: No Charge, Access+ HMO: $5 copay.
