HARTFORD SERVICES
As a covered employee under our Hartford Group Life insurance policy, you have access to services that can
help you during a disability or the loss of a loved one. For additional information, please contact the Human
Resources Department.
Following are brief descriptions of the services offered through Hartford:
• Ability Assist Counseling Services: If disability happens, you can receive up to three face-to-
face emotional or work-life counseling sessions per family member, per occurrence, per year. Legal and
financial counseling are also available by telephone during business hours.
• Beneficiary Assist Counseling Services: This service provides you, your eligible beneficiaries
and immediate family members with unlimited 24/7 phone access for legal advice, financial planning and
emotional counseling related to the death of yourself or a loved one.
• Estate Guidance: This service helps you create a simple, legally binding will quickly and conveniently
online, saving you the time and expense of a private legal consultation.
• Everest Funeral Planning and Concierge: Everest’s advisors help families who are planning a
funeral understand all of their options and put them into action while staying within their budget.
PLANSOURCE ONLINE ENROLLMENT
With the PlanSource Online Enrollment System, you and your family can access your benefits information
whenever you want, from home or any place where you have internet access. Use PlanSource to make your
benefit elections, update your personal information, and to locate important benefits documents.
To Enroll or Make Changes to Your Benefits:
Log in to www.plansource.com/login.
• Username: The first initial of your first name, up to the first six letters of your last name, and the last four
digits of your SSN.
• Password: Your birthdate in the YYYYMMDD format.
It is very important that you login to PlanSource and verify your personal information, elect or decline benefits,
and identify a life insurance beneficiary.
