Life and AD&D Insurance

Reliance Standard | Basic Life and AD&D Insurance
Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with the
company. Automated Motor Shades pays for coverage, offered through Reliance Standard, in the amount of $25,000. If your death
is due to a covered accident or injury, your beneficiary will receive an additional $25,000 through Accidental Death and
Dismemberment (AD&D) coverage.

You may update your beneficiary designation at any time throughout the year on Ease (see page 4).

Long Term Disability Insurance

Reliance Standard | Long Term Disability
Automated Motor Shades offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an
extended period of time, through Reliance Standard. If you become totally and permanently disabled, benefits begin 180 days after
the start of your illness or injury and will continue as long as you remain disabled up to retirement age. LTD works with state
disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal
to 60% of your pre‐disability earnings up to a maximum benefit of $6,000 per month.

Employee Assistance Plan (EAP): This plan provides 24/7 phone counseling services, and up to 3 face-to-face visits for you and
anyone in your household. The EAP counselors provide counseling for stress, financial stress, work/life matters, and more.
