ONLINE BENEFITS ENROLLMENT





                                                                                                                                    With bswift, you and your family can access your benefits information whenever you want--from home or any place where you have
                                                                                                                                    Internet access. Use bswift to make your benefit elections and locate important benefits documents. You will be able to access the Web
                                                                                                                                    Benefits site directly from the Paylocity Employee Self Service Portal by clicking on the “Applications”
                                                                                                                                    tab from Web Pay, and then selecting the “Enterprise Web Benefits” link from the drop-down options. You will not need a separate User
                                                                                                                                    ID or password when accessing the site via the Employee Self Service Portal.




                                                                                                                                                   ACCESSING BSWIFT OUTSIDE OF PAYLOCITY
                                                                                                                                                   Go to http://lyonlifebenefits.bswift.com.
                                                                                                                                                   •   Username: The first letter of your first name followed by your last name.
                                                                                                                                                   •   Password: The last four digits of your Social Security Number, as long as you did not change your password
                                                                                                                                                       during the 2017 plan year.




                                                                                                                                     The enrollment process consists of the following steps. You will be taken through each tab to make changes or confirm your information
                                                                                                                                     on file and choose your benefits for the new plan year.


                                                                                                                                     B       Review and Update Your Personal Information
                                                                                                                                             Before beginning your enrollment, please verify the accuracy of all of your personal information (e.g., address, DOB, etc.). If
                                                                                                                                             you notice any errors, please contact Human Resources. Changes can be made via the Employee Self Service Portal in Web
                                                                                                                                             Pay. Your changes will be updated in the Web Benefits system within 24 hours. Verify that all information is accurate. When
                                                                                                                                             done, check “I agree” at the bottom of the page and click “Continue.”


                                                                                                                                     C       Review and Update Your Family Information
                                                                                                                                             Please be sure to add all dependents in the Family Information section before proceeding to the next section. Click “Add
                                                                                                                                             Dependents.” When all of your family information is accurate, check “I agree” and click “Continue.”


                                                                                                                                     D       Enroll in Benefits
                                                                                                                                             To start your enrollment, on the “Enroll” tab, click on the “Get Started” button.

                                                                                                                                     E       Beneficiaries
                                                                                                                                             Once you have completed all elections, you will be asked to name beneficiaries for those plans applicable. Primary beneficiary
                                                                                                                                             information is required. You are also given the opportunity to name secondary beneficiary information. This is optional. All
                                                                                                                                             listed dependents will populate the beneficiary roster. All assignments must equal 100%. If you wish to add a beneficiary that is
                                                                                                                                             not a listed dependent, you may do so by clicking the “Add New Beneficiary” button and completing the requested information.

                                                                                                                                     F       Review Elections
                                                                                                                                             When you have completed your enrollments, review all of your benefit elections and covered dependents.

                                                                                                                                             Once you have completed your review, check the “I agree to ALL the changes I’ve made to my enrollment and I am done
                                                                                                                                             with my enrollment” boxes at the bottom of the page. Lastly, click on the “Complete My Enrollment!” button at the bottom
                                                                                                                                             of this page.
