Life and AD&D Insurance

Anthem | Basic Life and AD&D Insurance
Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with the
company. Tongal pays for coverage, offered through Anthem, in the amount of $25,000. If your death is due to a covered accident
or injury, your beneficiary will receive an additional amount through Accidental Death and Dismemberment (AD&D) coverage.

Supplemental Benefits

Humana | Accident Insurance
The Humana Accident insurance will pay you a benefit outlined in the benefit summary to help cover your out of pocket medical
costs. For covered accidental injuries, fixed benefits are paid directly to you regardless of your other coverages and you can spend
it any way you choose. Benefits are paid according to a fixed schedule that includes benefits for hospitalization, fractures and
dislocations, emergency room visits, and more.

Humana | Critical Illness Insurance
The Humana Critical Illness plan can help you and your family prepare for the financial stress a critical illness can cause. The plan
pays a lump sum benefit to help with expenses associated with the initial occurrence of a critical illness such as heart attack, stoke,
or cancer. Benefits are also available for your spouse and eligible children. The plan also offers a wellness screening benefit of
$150 per member per year. Please see the summary of benefits for additional information.

Employee Assistance Program

Anthem | Employee Assistance Program
The Employee Assistance Program (EAP) through Anthem provides you and your household members with free, confidential
assistance to help with personal or professional problems that may interfere with work or family responsibilities and obligations.
Services are available 24 hours a day, 7 days a week via a toll-free nationwide number. You and your household members can
receive up to 3 counseling sessions per person, per problem, per year.

Travel Assistance Program

Anthem | Travel Assistance Program
The Travel Assistance program, arranged by Anthem Blue Cross, provided by Europe Assistance USA, offers emergency assistance
when you and/or your family are traveling 100+ miles away from home or work. Travel Assistance is available 24 hours a day, 7
days a week with trained professionals to provide valuable support in a medical emergency, such as locating doctors, dentists and
facilities, setting up emergency transfers, sending travel companions, pets, and vehicles home, coordinating bedside visits, and
setting up and paying for the return of mortal remains (see contract for restrictions). In addition, emergency travel support services
are available and provide services such as immunizations (shots), passport needs, emergency payment and cash advances,
emergency messages, and legal help.
