MEDICAL PLAN ADMINISTRATORS
EBA&M
EBA&M is our Benefit Plan Administrator responsible for providing administrative services to Galasso’s Bakery’s employees for the
health care and prescription drug benefits. EBA&M is contracted to act on your behalf in administering our medical and
prescription benefit plans by:
• Issuing I.D. cards
• Maintaining eligibility
• Processing all claims and making claim payments to hospitals, doctors and facilities
• Answering your questions about how or when a claim is paid
• Answering detailed questions about your plan options and benefit coverage
Contacting EBA&M
• Website: Go to www.ebam.com. You are encouraged to register online. Once you register and log in to
the secure site you can:
• Check payment status of recently paid claims
• Find detailed health plan information
• Download claims forms
• Telephone: Call (800) 249-8440.
• Mobile App: Use EBA&M’s mobile app for iPhones and Android devices to access claim status and EOBs
at any time. You may also download a copy of your medical ID card. You must be registered with
EBA&M online to use the mobile app. Once registered, go to www.ebam.com from your mobile device.
From the home page, click on the Appple (for iPhones) or the Google Play (for Android) link to install the
app on your device.
MEDICAL INSURANCE
Network PPO | EBA&M (Anthem Blue Cross)
The Network Preferred Provider Organization (PPO) plan allows you to direct your own care. This plan requires that you only utilize
doctors, clinics, and hospitals that belong to the network, except in the case of an emergency, however, you may self-refer to
specialists. If you choose to see a provider who is not part of the network, you will be required to pay the full cost for your care.
Finding a Medical Provider
• Website: Go to www.ebam.com. Select “Plan Participants” and “Find a Provider”. Enter “Medical
(Employer-Sponsored) Blue Cross PPO (Prudent Buyer) Large Group. You will be redirected to the
Anthem Blue Cross website to complete your search criteria.
• Telephone: Call EBA&M at (800) 249-8440.
Summary of Benefits and Coverage (SBC)
Health insurance issuers and group health plans are required to provide you with an easy-to-understand summary about your
health plan’s benefits and coverage, referred to as a Summary of Benefits and Coverage (SBC). This guide is designed to help you
understand the medical plan options offered to you by Galasso’s Bakery. Please refer to the SBC and carrier contracts provided by
EBA&M (Anthem Blue Cross) for additional plan details.
