Temporary ID Cards

Medical Dental PPO
Temporary ID card will be available after 72 hours once No ID Card is needed. Please provide your dentist with
registered your Social Security Number

Step 1: Register on www.myuhc.com Vision
Step 2: Enter member ID # received from HR Step 1: Register on www.vsp.com
Department and Group #905826.
You may also use your Social Security Step 2: Under the Benefits & Claims tab, click on
Number and date of birth to register How to Use My Benefits
Step 3: Select Member Vision Card
Step 3: Create Username and Password You may also provide your eye doctor with your Social
Step 4: Enter email address and choose Security Security Number to access coverage
Questions, and click Submit
Step 5: Under My Account Settings, click Temporary
