         Disability Insurance


         California State Disability
         If  you  experience  a  Short  Term  Disability  claim,  you  are  eligible  for  benefits  provided  by  the  state.  California  State  Disability
         Insurance (SDI) is a partial wage-replacement insurance plan for California workers. The SDI program is state-mandated and funded
         through employee payroll deductions. SDI provides short term benefits to eligible workers who suffer a loss of wages when they
         are unable to work due to a non-work related illness or injury, pregnancy or childbirth. For more information on eligibility, benefit
         amounts and instructions on how to file a claim for the SDI program, please visit the EDD website at www.edd.ca.gov.

         Mutual of Omaha | Voluntary Short Term Disability Insurance

         Alexander’s Mobility Services offers you the opportunity to purchase Voluntary Short Term Disability (STD) income replacement at
         discounted group rates, through Mutual of Omaha. If you experience a temporary disability, benefits begin 7 days after the start of
         your accident, sickness or pregnancy and will continue up to a maximum of 12 weeks. STD works with state disability programs,
         Social Security, and any other group disability coverage, to provide you with a combined weekly benefit equal to 66-2/3% of your
         pre‐disability earnings up to a maximum benefit of $1,000 per week.

         Mutual of Omaha | Long Term Disability Insurance
         Alexander’s Mobility Services offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an
         extended period of time, through Mutual of Omaha.
         •   Option 1 Base Plan (Employer Paid): If you become totally and permanently disabled, benefits begin 90 days after the
            start of your illness or injury. This plan works with state disability programs, Social Security, and any other group disability
            coverage, to provide you with a combined monthly benefit equal to 50% of your pre‐disability earnings up to a maximum
            benefit of $1,000 per month.
         •   Option 2 Buy-Up Plan (Employee Paid): If you become totally and permanently disabled, benefits begin 90 days after
            the start of your illness or injury. LTD works  with  state  disability programs, Social Security, and any other group disability
            coverage, to provide you with a combined monthly benefit equal to 60% of your pre‐disability earnings up to a maximum
            benefit of $5,000 per month.

         Travel Assistance


         Worldwide Travel Assist | Mutual of Omaha
         Alexander’s Mobility Services provides a Worldwide Travel Assistance Plan to you at no cost. This program, arranged by Mutual of
         Omaha, offers emergency assistance for up to 120 days in length when you and/or your family are traveling 100+ miles away from
         home or work. Worldwide Travel Assistance is available 24 hours a day, 7 days a week with trained professionals to assist you with
         pre-trip assistance, to provide immediate attention for  emergencies while traveling, and for emergency travel support services
         including:
         •   Telephonic translation and interpreter services – 24/7 access to telephone translation services
         •   Locating legal services – referrals for local attorney or consular offices and help maintain business and family communications
            until legal counsel is retained (includes coordination of financial assistance for bonds/bail)
         •   Baggage – assistance with lost, stolen or delayed baggage while traveling on a common carrier
         •   Emergency payment and cash – assistance with advance of funds for medical expenses or other travel emergencies by
            coordinating with your credit card company, bank, employer, or other sources of credit; includes arrangements for emergency
            cash from a friend, family member, business or credit card
         •   Emergency messages – assistance with recording and retrieving messages between you, your family and/or business associates
         •   Document replacement – coordination of credit card, airline ticket or other documentation replacement
         •   Vehicle return – if evacuation or repatriation is necessary, return your unattended vehicle to the car rental company
