With the Ease Central Online Enrollment System, you and your family can access your benefits information
whenever you want, from home or any place where you have internet access. Use Ease Central to view plan
details, coverage amounts and costs.

To Enroll or Make Changes to Your Benefits

• Username & Password: You will receive an email with information that is unique to you.
• Log in to Ease Central - click on www.fpch.ease.com

Once you are logged into the website, follow the prompts on each page to complete your benefit enrollment.
You will be asked to verify that your personal information is correct and enter in any of your dependent infor-
mation.

For optimal performance, it is recommended that you use Chrome or Firefox as your internet browser.
Be sure to save Ease Central as a favorite in your web browser!

Annual First Presbyterian Church of Hollywood plans are
partially arranged by FCPH and governed by it’s
plan rules and documents. ERISA and various other
Notices state and federal laws require that employers
provide disclosure and annual notices to their plan
participants. FCPH distributes annual notices to
new-hires, and each year during open enrollment.

• Medicare Part D Notice of Creditable Coverage posted such as the Summary of Benefits and
• HIPAA Notice of Privacy Practices Coverage (SBC), annual notices, carrier benefit
• Women's Health and Cancer Rights Act (WHCRA) summaries, evidence of coverage booklets,
• Newborns’ and Mothers’ Health Protection Act claim forms, and much more. These documents
• Special Enrollment Rights include detailed information about the FPCH
benefit plans and can help you plan for
• Medicaid & Children’s Health Insurance Program upcoming services. From work or home, 24
hours a day, 7 days a week, you and your
eligible dependents may access Ease Central.
