BENEFITS

You may purchase individual policies from Unum and Cancer Guardian at discounted group rates. These policies offer
direct-to-the-policyholder cash payouts to help cover what other insurance doesn’t. Should you elect these plans, your
premiums will be taken through payroll deductions on a pre-tax basis.

Unum Voluntary Accident Off-Job
Accident insurance provides a lump-sum benefit to you, your spouse, or children after a covered injury or accident.

Unum Critical Illness (including Cancer)
This plan pays a lump sum benefit to you, your spouse, or children at the diagnosis of a covered critical illness. You can
choose to purchase $5,000 to $50,000 of coverage, and you can use your benefit payments any way you see fit.

Unum Individual Short Term Disability
Short Term Disability can pay you a percentage of your monthly salary if you become injured or ill due to a covered off-the-
job disability or covered pregnancy.

Unum Whole Life Insurance
Whole Life Insurance is offered to you, your spouse, and children. In addition to a death benefit, this plan provides a living
benefit that you can use during times of need. Unlike our company-provided Term Life insurance, you can keep your Whole
LIfe coverage if you leave the company or retire.
