VOLUNTARY BENEFITS
MetLife | Group Accident
Group Accident insurance pays benefits to help you cover the unexpected costs associated with a covered accident. Benefits are
flat dollar payments by incident (not based on medical expenses incurred). Covered benefits include specified injuries, medical
treatments, hospital admissions/confinements, accidental death, dismemberment, and loss/paralysis. Coverage is also available
for your spouse/registered domestic partner, and your dependent children.
Health Screening Benefit (Wellness): MetLife will also provide an annual benefit of $75 for taking one of their eligible screening/
preventive measures.
MetLife | Critical Illness
Critical Illness insurance is designed to help you offset the costs of certain covered critical illnesses such as heart attack, stroke,
kidney failure, coronary artery bypass graft, and cancer. When diagnosed with a covered condition, you will receive a lump-sum
benefit that you can use to pay for expenses such as deductibles, copays, transportation, personal expenses, and so on. A
reoccurrence benefit is available if an initial benefit has been paid for the covered condition.
Health Screening Benefit (Wellness): MetLife will also provide an annual benefit of $75 for taking one of their eligible screening/
preventive measures.
Critical Illness is available to you, your spouse/domestic partner, and your children. You may purchase coverage for yourself in the
amount of $15,000 or $30,000 – and you will not be asked to submit a medical questionnaire or take a medical exam. If elected,
your dependent coverage will be 50% of your initial benefit.
MetLife | Hospital Indemnity
The Hospital Indemnity plan pays a benefit if you are admitted to the ICU, and an additional benefit if you are confined to a
hospital – whether for planned or unplanned reasons. You can use your benefit payment as you wish. Use it to help cover your out
-of-pocket medical expenses, or for whatever purpose you choose. This benefit is available for you, your spouse/domestic partner,
and your children. You have a choice of two levels of coverage through MetLife’s Low or High plans.
Health Screening Benefit (Wellness): MetLife will also provide an annual benefit of $50 for the low plan, $100 for the high plan for
taking one of their eligible screening/preventive measures.

With these plans, you can receive legal consultation, legal document review, debt collection assistance, trial defense services,
ticket resolution, civil lawsuits and many others for an unlimited number of matters. . This plan does not cover divorce, DUIs or

examples of covered legal matters. Your legal plan covers a broad range of legal matters, which may include:
Payroll deductions are $19.50 per employee per month which covers spouse and dependents.
Voluntary Benefit Advisors (VBA) | MetLife Assistance
Employees will call Voluntary Benefit Advisors Service Center using the phone number referenced below to speak with a benefits educator.
The benefit educator will be able to answer questions you have about all of the benefits offered to you and answer questions about the
MetLife voluntary coverages. VBA will provide claims assistance for your MetLife Accident, Critical Illness and Hospital plans.
