Life Insurance

Anthem | Basic Life and AD&D
Life insurance protects your family or other beneficiaries in the event of your death while you
are still actively employed with the company. Westmark School pays for coverage, offered
through Anthem, in the amount of 2x your annual salary up to $400,000. If your death is due to
a covered accident or injury, your beneficiary will receive an additional amount through
Accidental Death and Dismemberment (AD&D) coverage.

Guardian | Voluntary Life
In addition to the company provided Basic Life and AD&D benefits, you may elect to purchase
Voluntary Term Life insurance at group rates provided by Guardian. You pay for this coverage

Consider updating Employee: You may purchase Voluntary Life insurance coverage for yourself in increments of
your beneficiary $10,000 up to a maximum of $300,000. Guarantee issue is available during the initial eligibility

Disability

Guardian | Short Term Disability
Westmark School offers you Short Term Disability income replacement if you are disabled due
to accident, sickness or pregnancy. If you experience a temporary disability, benefits begin 15
days after the start of your accident, sickness or pregnancy. Short Term Disability is reduced by
California State Disability, Social Security, and any other group disability coverage, to provide
you with a combined monthly benefit equal to 66 2/3% of your pre-disability earnings to a
maximum of $1,386 a week.

Guardian | Long Term Disability
Westmark School offers you Long Term Disability (LTD) income replacement if you are disabled
for an extended period of time. If you become totally and permanently disabled, benefits begin
180 days after the start of your illness or injury. Long Term Disability is reduced by California
State Disability, Social Security, and any other earnings, vacation pay or group disability, to
provide you with a combined monthly benefit equal to 60% of your pre-disability earnings to a
maximum of $6,000 a month.
