You may enter anything your organization wishes to enter into the Container Description, Container
Reference, and Invoice Header fields. Each of these items is limited to 255 characters.
Enter a description of the container, such as a Site ID.
Enter a reference comment for the container, such as an Outreach ID.
Sellectt ((orr add)) Requesttorr,, Conttactt,, and Fax Numberr as usuall..
Select Billing Type as usual.
Select Delivery Type as usual.
Use the listed per patient rate or enter a new rate for this Container.
Enter a header that will actually appear on the Invoice to the Requestor.
Review the summary as usual.
Save and return to results list.
Save and go to next screen.
Return to results list without saving.
