Program Registration Waiver and Consent Policy
DEADLINES Registration is on-going; however, review each program listing to con rm registration deadline. Most programs have a maximum number of participants and registration for those programs may close prior to the deadline. Registration deadlines do not guarantee a spot in the program.
CONFIRMATIONS/CANCELLATIONS/LOCATION CHANGES
The Recreation and Education Department does not mail con rmation of enrollment for any program; however, we will make every effort to notify all participants if a program has been cancelled or changed for any reason.
NON-ENDORSEMENTPOLICYInstructorsusedbytheRecreationandEducation Department are contracted or hired because they have a special skill and talent, or area of expertise that they are willing to share with others. The Recreation and Education Department does not endorse the opinions or business af liations of our instructors/contractors.
PHOTO RELEASE By registering for any Recreation and Education program, you agree to allow publication of photos or lm taken at any program, event or facility associated with the Recreation and Education Department. Photos and/or lm may be used in print or broadcast form for materials relating to our programs and activities. Participants and/or spectators will not be compensated for these photos or videos.
LAKEWOOD YOUTH RECREATION SCHOLARSHIPS Scholarships areavailableforLakewoodyouthtoparticipateinrecreationalprograms.Eligibilityisdeterminedby family income. Veri cation of income may be required. Applications are available at the Recreation and Education Department.
FACILITY PERMITS The Recreation and Education Department, on behalf of the Lake- wood Board of Education, provides public rental of school facilities, during non-school hours, such as gyms, classrooms, cafeterias, etc. The of ce also issues permits for baseball/softball elds, tennis courts and other outdoor facilities. Outdoor facility permits are available May 1 through September 30. For more information and rates, please call the Recreation and Education Department.
ADULT PROGRAMS All Adult programs are for participants 18 years and older unless otherwise stated.
PAR TICIPAN TS WI TH DIS ABILI TIES Anyone needing assistance to participate in any program should contact the Recreation and Education Department so that reasonable accommodations can be made to ensure a positive experience for all programs.
PROGRAM REFUND POLICY
CANCELLED PROGRAMS The Recreation and Education Department reserves the right to cancel a program with fewer than the required number of registrants. In the event that the Recreation and Education Department cancels a program, a full refund will be granted to all individuals enrolled in the program.
REFUNDS BEFORE PROGRAM BEGINS If a participant wishes to withdraw from a program, the participant must submit a written request at least 7 business days prior to the rst session of the program, unless otherwise noted, provided it does not reduce the participation level below the required minimum*. A $15 processing fee will be assessed. Participants also have the option of receiving a credit on their Lakewood Recreation account. A $5 processing fee will be assessed. Lakewood Recreation and Education account credits may be used for any program offered by the Recreation and Education Department.
REFUNDS AFTER PROGRAM BEGINS In the event that a participant wishes to withdraw after the rst session of the program and before the second session of the program, a written request must be submitted before the second session. A participant will receive 50% of the registration fee provided it does not reduce the participation level below the required minimum*.
NO REFUNDS No refunds will be issued after the second session of any program or if a program registration fee is $15 or less. On-line service fee is nonrefundable.
HOW REFUNDS ARE ISSUED If the registration fee is paid by cash, check or money order the participant will receive a refund in the form of a check. If the registration fee is paid by MasterCard or Visa, the participant will receive a credit to the corresponding account. There will be no cash refunds. A Program Refund Request Form MUST be completed for all refunds. NO EXCEPTIONS. Program Refund Request Forms are available online at www.lakewoodrecreation. com or at the Recreation and Education Department.
*In certain circumstances refunds cannot be granted because tickets, supplies and/or instructor fees are determined and/or purchased in advance based on enrollment numbers.
CANCELLED PROGRAM: The Recreation and Education Department reserves the right to cancel any program that does not meet the minimum number of required registrants. In the event the Recreation and Education Department cancels a program, a full refund will be granted to all registrants. PARTICIPATION: By registering for a program, I acknowledge the possibility that injuries may occur during participation in Recreation and Education programs and expressly assume the risk of all such injuries. Although most injuries are minor, injuries may occur that are severe enough to cause permanent disability, such as paralysis, or death. In consideration of the Recreation and Education Department allowing me or my child, as applicable, to participate in Recreation and Education programs, I hereby irrevocably and unconditionally release and forever discharge the Lakewood Recreation and Education Department, Lakewood Board of Education and their board members, administrators, employees, agents, instructors, aides and volunteers (collectively, the “Released Persons”) from any and all claims, actions, demands, rights, liabilities, damages, costs, expenses or causes of action of whatever kind which may arise out of or relate in any way to my or my child’s participation in any Lakewood Recreation and Education program. Further, I hereby indemnify, defend and hold harmless any and all Released Persons against and from any and all claims, actions, demands, rights, liabilities, damages, costs, expenses or causes of action of whatever kind which may arise out of or relate in any way to my or my child’s participation in any Recreation and Education Department program or any use of my or my child’s recordings as authorized below. I understand that all participants in Recreation and Education Department programs are required to abide by the rules and policies of the Lakewood BoardofEducationandtheRecreationandEducationDepartment,andfurtherunderstandthatmy or my child’s failure to do so may result in termination of my or my child’s participation in any or all programs. I will be responsible for all facilities and equipment used and/or issued to me or my child throughout the programs, and will pay the repair/replacement cost for any facility and/or equipment damaged, lost or not returned by me or my child. I understand that all facilities and equipment must be used only in the manner for which they are intended.
FEES & REGISTRATION INFORMATION: All participants must be registered and fees paid in full prior to the start of any program. I hereby certify that all information provided for registration is accurate and understand that providing false information may result in my inability to participate in any Lakewood Recreation and Education Department program.
YOUTH ATHLETIC REQUIREMENTS: Prior to participating in any youth athletic league, the following must occur:
·The participant MUST have accidental insurance coverage that will pay his/
her medical or surgical expenses associated with any injury, major or minor,
that results from his/her participation in any athletic league.
·All participants must have a current physical on le at the Recreation and Education Department
at the time of registration. The physical must speci cally clear the player to participate in sports. The physical must be current for the duration of the season. All Players must submit a current physical.
·All participants must have a birth certi cate on le at the Recreation and Education Department at the time of registration. Birth certi cates kept on le at the Recreation and Education Department until the child turns 18 and may be disposed of sooner if determined it’s no longer needed or at the parents request.
USE OF RECORDINGS: I hereby authorize the Lakewood Board of Education and the Recreation and Education Department, their employees and agents to record my or my child’s picture and/or voice (including but not limited to my or my child’s spoken or other vocal content) on photographs, lms, video and audio tapes, CD-ROMs, DVDs, digital les or any other media in connection with any or all Recreation and Education programs. Further, the Lakewood Board of Education and the Recreation and Education Department are hereby authorized, at their discretion, to use and to license others to use such recordings in their original or edited form in any media (print, broadcast and others) for any and all educational and/or commercial purposes, and to use my or my child’s name, likeness, voice and spoken or other vocal content for such purposes. I acknowledge and agree that neither I nor my child will be compensated for any use by the Lakewood Board of Education or the Recreation and Education Department of such recordings. I further acknowledge and agree that the Lakewood Board of Education will own exclusively any and all rights to such recordings.

*If Lakewood City Schools are closed for inclimate weather, all programs scheduled within the schools that day will not take place. Every effort will be made to make those classes up.
