               Dear Mr. Kapon:

               Your Medicare evaluation is attached. You might begin by checking the information on the
               client data sheet on page 5 to make sure it is correct. If there is anything that needs to be
               changed, let me know.

               In most cases the costs shown in the evaluation are estimates for the 2018 plan year, even
               though you will also be enrolled for for the last two months of 2017. Also, Medicare’s cost-
               sharing amounts (Part A and Part B deductibles) shown on pages 6-7 are the 2017 numbers –
               Medicare has not yet released the cost-sharing amounts for 2018.

               The evaluation compares four options -- two Medigap plans (Plan F and Plan L) and two
               Medicare Advantage PPO plans. Both Advantage plans include Part D coverage, and if you
               decide to enroll in either of them you will not need any further supplemental coverage.

               Medigap policies, on the other hand, do not include prescription drug coverage, and so if you
               choose a Medigap policy you will also need to enroll in a stand-alone prescription drug plan.
               Even though you do not currently take any Rx drugs, you are required to have Part D coverage
               or else you could be subject to lifetime penalties. Your Rx drug coverage options are described
               later.

               After you have decided which type of coverage you want, you can enroll by telephone. Toll-free
               numbers for the plans and Nevada insurance companies that sell Medigap policies are included.
               Whichever plan(s) you call to enroll, be sure that the person you speak with knows that you
               want your coverage to begin November 1.

               In making your coverage choices, it’s important to understand the guaranteed issue rules for
               Medigap policies. Individuals who purchase a Medigap policy during the first six months that
               they have Part A and Part B cannot be declined coverage or charged more because of pre-
               existing conditions. They can’t even be asked questions about their health, other than their age,
               gender, and whether they smoke.
