               Dear Ms. Evans:

               Your Medicare evaluation is enclosed. In most cases the costs shown in your evaluation are for
               the 2018 plan year, even though you will be enrolled only for the last five months of the year.
               In your questionnaire you indicated that you prefer to hav a Medigap policy, and that you
               would like to see the high-deductible Medigap Plan F (HDF) included in the evaluation. The
               evaluation, then, compares four Medigap plans (Plans F, G, N, and HDF).

               Once you have decided which Medigap plan you want and the company you will buy it from,
               you can purchase it directly from the insurer, stipulating that you want the policy to become
               effective on August 1. As you probably know, people who purchase Medigap policies must also
               enroll in Part D stand-alone plans, discussed in the Rx drug coverage section below. To enroll in
               a Part D stand-alone plan, you can either call the plan’s toll-free number shown on page 4 of
               the evaluation or you can call 800-MEDICARE.

               Since you’re going to get a Medigap policy, it may help to review features that all these policies
               share (you may already be aware of these). Medigap policies can be purchased during the first
               six months you have Part A and Part B without your having to answer health-related questions.
               But after that period has past, in Arizona and most other states, you will have to answer
               questions about your health before you can get a policy. Consequently, after the initial six-
               month guaranteed-issue period is past, people who have serious health problems may not be
               able to purchase a Medigap policy or upgrade to a more comprehensive plan.

               Perhaps the most desirable attribute of Medigap policies is the flexibility they offer. There are
               no networks, and you will be covered when you see any provider who accepts Medicare (as
               some 98% of doctors and public hospitals do). That means you can go to virtually any doctor,
               hospital or clinic in the U. S. and be covered.

               Medigap policies do not cover routine dental and vision care or hearing aids. All four plans in
               your evaluation include some benefits for medical emergencies while traveling outside the
               United States (as shown on pages 6-7). Following are summaries of these plans, listed in the
               order of their comprehensiveness.
