If you change your Advantage plan coverage to the Health Encompass 65 HMO plan, you will
               want to enroll in the lowest-cost stand-alone plan for the Rx drugs that you take. According to
               the Medicare Plan Finder, that is the Aetna Medicare Rx Saver plan. Shown below are the
               estimated 2018 costs for this Aetna plan and your current plan.

               You can see a more detailed comparison of your current plan’s and the Aetna Medicare Rx
               Saver’s plan coverage in Appendix C2.

               Some prescription drug plans (both stand-alone and Advantage) have preferred pharmacies
               where you can get lower prices when you get retail refills. In your current plan, there aren’t any
               preferred pharmacies, but in the Aetna Medicare Rx Saver Plan there are preferred pharmacies,
               including CVS, shown on the last page of the Appendices.

               Changing Plans

               Prior to changing plans, you should verify that your physicians are in the network of the plan
               you want to enroll in. Probably the simplest way to do this is to call your current plan’s phone
               number and tell them you want to switch to another Advantage plan that’s also sponsored by
               the Independent Health Association.

               But before making this switch, ask the representative to confirm that your doctors are in your
               new plan’s network. The representative will likely say that the networks are the same for the
               plan you are leaving and the plan you are changing to. But if the rep doesn’t know, you may
               have to give him/her the names of your doctors so that he/she can look them up to verify that
               they are in the plan you will be changing to. Appendix A2 indicates that all of your doctors are
               in the network of the Health Encompass 65 HMO plan that you want to transfer to.

               If for some reason it turns out that one or more of your doctors is NOT in the new (new to you,
               at least) plan’s network, you will want to remain in your current plan. In that case you can still
               save money by changing to mail-order refills.

               If all your doctors are in your new plan’s network, you call the Aetna Medicare Rx Saver plan
               and enroll with an effective date of 1-1-18 (the enrollment phone number is shown on page 4
               of your evaluation). As noted, you need to do this by December 7.
